About this role
We are seeking a compassionate and dedicated Registered Nurse to provide in-home healthcare services. The nurse will conduct patient assessments, develop and implement care plans, educate patients and caregivers on medication management, coordinate care with physicians and interdisciplinary teams, and ensure accurate documentation of all visits and treatments. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in patients' homes.

- Educate patients on health management, safety, and treatment protocols.
- Coordinate with physicians, caregivers, and interdisciplinary teams to ensure seamless care delivery.
- Document all patient interactions, assessments, interventions, and outcomes accurately and timely.
- Maintain compliance with healthcare regulations and agency policies.
- Manage scheduling of visits efficiently to meet patient needs.
Qualifications
- Valid RN license in the state of practice.
- Valid driver’s license with reliable transportation.
- Home health or field nursing experience preferred.
- Strong clinical judgment and critical thinking skills.
- Excellent documentation and communication skills.
- Ability to work independently in a home environment.
