We're growing and expanding our service areas, and we're looking for multiple RNs to join our Home Health team , with a current focus on Buckley, Puyallup, and Graham .

What You'll Do

As a Home Health RN, you'll have the opportunity to build meaningful relationships with patients while providing skilled care where they're most comfortable: at home.

  • Assess patients in their homes and identify their individual care needs
  • Develop and manage personalized plans of care in collaboration with providers
  • Provide skilled nursing care, education, and support to patients and families
  • Coordinate care with therapists, physicians, and other members of the interdisciplinary team
  • Monitor patient progress and adjust care as needs change
  • Complete accurate, timely clinical documentation
  • Participate in quality initiatives that help us continually improve patient care

What You'll Bring

  • Current Registered Nurse (RN) license
  • Current AHA BLS certification
  • Home Health and/or Hospice experience preferred
  • Valid driver's license, insured vehicle, and reliable transportation
  • Ability to work independently while collaborating with a clinical team
