About this role

*Overview*

We are seeking a compassionate and dedicated nurse Home Health LPN to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support and medical attention needed for optimal health outcomes.

The nurse must have experience with G-tube care/tube feedings and tracheostomy care. The ideal candidate will have a strong nursing background, excellent communication skills, and the ability to work independently while safely managing medically complex patients in the home setting.

  • Conduct comprehensive patient assessments and develop individualized care plans based on patient needs.
  • Administer medications and treatments as prescribed.
  • Perform G-tube care and administer tube feedings, including monitoring for complications and maintaining tube-site integrity.
  • Perform tracheostomy care,including routine trach maintenance, assessment, suctioning as ordered, and monitoring respiratory status.
  • Monitor vital signs and document changes in patient condition accurately.
  • Provide education and support to patients and families regarding medications, G-tube/tube feeding management, tracheostomy care, and disease management.
  • Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
  • Maintain accurate and timely documentation using an electronic health record (EHR) system.
  • Assist with activities of daily living (ADLs) and mobility support as appropriate.
  • Evaluate patient progress and promptly report significant changes in condition to the appropriate provider.

*Skills & Qualifications*

  • Active LPN license.
  • G-tube/PEG tube care and tube feeding experience required.
  • Tracheostomy care and suctioning experience required.
  • Strong assessment and medication administration skills.
  • Familiarity with home health nursing and emergency protocols.
  • Ability to work independently and appropriately respond to changes in patient condition.
  • Excellent communication and documentation skills.
  • Experience caring for medically complex patients preferred.
