Acnora Compassionate Care is seeking a caring and dependable Licensed Practical Nurse (LPN) to join our hospice team and work on a PRN basis. The LPN works under the direction of an RN and the interdisciplinary team to provide high-quality nursing care, symptom management, and emotional support to hospice patients and their families in homes and/or facility settings. Must have good communication skills. Must be a team player with knowledge working with an electronic medical record.

Key Responsibilities

Provide direct nursing care to hospice patients in accordance with the plan of care
Monitor and document patient condition, symptoms, and responses to treatment
Assist with pain and symptom management, comfort measures, and personal care
Educate patients and families on disease process, medications, and care techniques
Communicate changes in patient status promptly to the RN case manager
Support patients and families emotionally during end-of-life care
Maintain accurate and timely clinical documentation
Collaborate with the interdisciplinary hospice team (RN, Hospice Aide, Social Worker, Chaplain, etc.)
Comply with hospice policies, procedures, and state/federal regulations

Pay: $25.53 - $29.13 per hour
