About this role
Acnora Compassionate Care is seeking a caring and dependable Licensed Practical Nurse (LPN) to join our hospice team and work on a PRN basis. The LPN works under the direction of an RN and the interdisciplinary team to provide high-quality nursing care, symptom management, and emotional support to hospice patients and their families in homes and/or facility settings. Must have good communication skills. Must be a team player with knowledge working with an electronic medical record.

- Provide direct nursing care to hospice patients in accordance with the plan of care
- Monitor and document patient condition, symptoms, and responses to treatment
- Assist with pain and symptom management, comfort measures, and personal care
- Educate patients and families on disease process, medications, and care techniques
- Communicate changes in patient status promptly to the RN case manager
- Support patients and families emotionally during end-of-life care
- Maintain accurate and timely clinical documentation
- Collaborate with the interdisciplinary hospice team (RN, Hospice Aide, Social Worker, Chaplain, etc.)
- Comply with hospice policies, procedures, and state/federal regulations
Pay: $25.53 - $29.13 per hour
