About this role
Golden Castle Adult Day Health Center is a licensed community-based adult services program (CBAS) that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities in a coordinated program of professional and compassionate services in a community-based group setting. Full-Time or Part-Time, Monday through Friday, no evening hours. DUTIES AND
Responsibilities
Provide a comprehensive nursing assessment of participants upon admission. Development and implementation of person-centered nursing care plans and participant assessments using evidence-based nursing practices and interventions. Participate in discharge planning from hospitals and/or Skilled Nursing Facilities.
Collect data and provide focused assessment on a quarterly basis. Identifies participant care requirements by establishing personal rapport with participants. Establishes a compassionate environment by providing emotional, psychological, and spiritual support to participants, friends, and families.
Promotes participant independences by establishing participant care goals; teaching participant, friends and family to understand condition, medication, and self-care skills; answering questions. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against participant care goals and standards, making or recommending necessary adjustments. Builds collaborative relationships.
