About this role
Lakes Dialysis Center is looking for a highly motivated candidate to join our team. The Dialysis RN will be under the supervision of the RN Manager and Director of Nursing, while completing designated tasks. We pride ourselves in careful selection of our team members, in order to provide the best quality of service to our patients.

Responsibilities
*
- Responsible for the support of and the delivery of all company policies and procedures
- Promote and assist in the maintenance of a safe and clean working environment.
- Provide safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the facility, state, and federal agencies.
- Apply your "core" nursing skills to assess, diagnose and implement complete care plans as you look at the whole patient.
- Responsible for reporting all new or unusual incidents, information, complaints, or problems to the Head Nurse using proper channels of communication.
- Responsible for direct patient care of assigned patients.
- Provide treatment technique for assigned patients as prescribed. This includes all aspects of hemodialysis procedure.
- Responsible for accurate documentation of information related to the patients treatment. The information is documented appropriately in the individual patient record.
- Responsible for reporting any significant information or change in patient condition to the Head Nurse.
- Responsible for educating the patient regarding end stage renal disease, diet, and medications.
- Familiarity with all emergency operational procedures.
- Lead, coach and guide with your nursing skills.
- Additional responsibilities assigned by Home Program Manager.

- Capable of operating all dialysis related equipment safely and efficiently.
- Capable of safely and efficiently operating all emergency equipment.
- Familiarity with all emergency equipment and ability to operate the equipment.

Qualifications
*
- Graduation from an accredited school of nursing.
- Current licensure as an RN by the state in which the dialysis facility is located.
- Demonstrated ability to function in a leadership position and to perform in new and emergent situations with sound judgment.
- Demonstrate a working knowledge of the English language and communicate verbally and in writing.
- Maintains the state mandated competency in Basic Cardiac Life Support, HIV basic course and yearly updates.
- Previous Chronic Hemodialysis experience
- Capable of operating all dialysis related equipment safely and efficiently.
*Knowledge and Experience*
- Must be fully bilingual in English and Spanish
- Minimum of one year chronic dialysis experience in Nursing Home/SNF environment.
- Data entry experience experience is preferred.
- Ability to adapt to company software, and appplications
Schedule will be: Monday-Wednesday-Friday
