Responsibilities: Schedule: Monday - Friday, within the core clinic hours of 0600 to 1800; While normal clinic hours are 0730-1630 Shift Structure: Daily shifts will be assigned by the Government supervisor to meet clinic and patient care needs. No Holidays/No Weekends/No Call General Knowledge: Telehealth knowledge/familiarity Knowledge of computer operations and proficiency in use of basic word processing, data entry and automated medical records. Knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, acknowledgement of teaching, and follow up care. Knowledge of disease management and population health Knowledge of preventive health schedules, procedures, disease management, population health principles, processes and education methods. Knowledge of a wide range of medical disorders/conditions and disease processes across the lifespan, as appropriate. Knowledge of professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing pediatric, adolescent, adults, obstetrics, and geriatric patients, as appropriate. Utilizes the nursing process as a basis for professional Job Specific Details and Tasks: Apply evidence-based, telehealth-specific interventions to achieve agreed upon outcomes during virtual encounters according to state regulations, regulatory agency standards, and appropriate organizational policies and procedures Knowledge of professional care theories, principles, practices, and procedures to advocate for the health needs of individuals and diverse populations across the lifespan and continuum of care. Utilizes the nursing process as a basis for professional Strongly preferred working knowledge of the military health care system including TRICARE health benefits or a working knowledge of the principles of fee-for-services, preferred provider network, and managed care/health management organizations is highly Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use to include knowledge of the administrative requirements for proper documentation of patients’ condition including disease progress, acknowledgement of teaching, and follow up care. Apply evidence-based decision support tools, instruments, and other resources relevant to the provision of nursing care utilizing telehealth technology, critical thinking, and clinical Using clinical algorithms, protocols or guidelines to systematically assess and determine patient's perception of his/her immediate needs and concerns, identifying the patient's desired course of action. Use professional nursing judgment to supersede triage decision support tool recommendation when clinically indicated and error on the side of caution if in doubt when determining disposition during triage encounters. Collaborate with other healthcare professionals in the provision of patient Facilitate the achievement of optimal outcomes in relation to clinical care, quality, and cost effectiveness. Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. Completes orientation and competency verification programs in accordance with (IAW) unit guidelines. Completes initial and stays up to date with annual trainings Participates in closing care gaps through proactive patient engagement and adherence to clinical best practices emphasizing preventive care and timely follow ups. Provide health promotions, counseling, and Performs a full range of nursing procedures such as: medication administration, injections, wound care, intravenous fluid administration, oxygen administration, vital signs, specimen collection, and other unit specific procedures as prescribed by the Care provided in coordination with other healthcare Recognize adverse signs and symptoms and quickly react in emergency Function independently in a highly collaborative environment, maintaining personal professional responsibility for assessing all symptom-based encounters. Make referral appointments and arrange specialty care as Conduct research in support of evidence-based practice to improve practice and patient Document telehealth interventions and services provided in the patient record, including modifications, changes, or omissions of identified plan. Qualifications: Minimum Qualifications: * Education: BSN Degree. Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE). * Experience: The nurse shall have at least 12 months of nursing experience within the last 24 months. * Licensure: Possess current unrestricted RN License from any state. * Life Support Certification: Possess current ARC or AHA BLS and Advanced Cardiac Life Support (ACLS) or Advanced Life Support (ALS) * Security: Possess the ability to pass a government background check/security clearance.
