Position

Summary

We are seeking a compassionate and experienced Registered Nurse (RN) to provide weekend home health services across the South County region . This role involves planning, organizing, and coordinating care, with a strong emphasis on community health and patient education.

Responsibilities

Develop and initiate individualized plans of care in collaboration with patients, caregivers, and physicians

Deliver preventative and rehabilitative nursing procedures as ordered

Educate patients and caregivers on health management and care techniques

Identify and implement discharge planning needs

Coordinate necessary equipment and services for patient care

Supervise LPNs/LVNs and Home Health Aides

Requirements

Active California RN license

Minimum 2 years of direct patient care

Minimum 1 year of home health experience required

Current CPR certification

Valid driver’s license and insured, reliable vehicle
