About this role

Monday- Friday 8:30am-4:30pm

Responsibilities

  • Monitor and record patient vital signs, symptoms, and medical history.
  • Collaborate with healthcare team members to develop and implement patient care plans.
  • Assess wounds, do treatments and documentation.
  • Educate patients and their families on disease management, medication administration, and self-care techniques.
  • Document patient care activities accurately and in a timely manner using electronic health records.
  • Adhere to infection control protocols and maintain a safe and clean environment.

Qualifications

  • Valid Licensed Practical Nurse (LPN) license in Connecticut
  • Prior work experience in wound care a plus
  • Great communication and people skills
  • Experience working with geriatric patients
  • Ability to lift and move patients safely using proper body mechanics
