Dependable Nursing is seeking an experienced, compassionate RN Case Manager to coordinate and oversee skilled home health care for patients across the lifespan — from pediatrics through senior care. This role begins as a part-time, hourly position and is expected to ramp quickly to full-time as the territory grows. The ideal candidate is a confident, self-directed clinician who is proficient in OASIS, thrives in the field, and is excited to help build a growing home health program.
Key responsibilities include conducting comprehensive in-home assessments and completing accurate, timely OASIS documentation at start of care, recertification, resumption of care, transfer, and discharge, and completing start-of-care visits and assessments.
