Provides assistance to the Director of Patient Care Services and Clinical Field Staff to ensure provision of quality patient care and effective team performance to meet company goals in accordance with company’s philosophy, policies, mission, and vision, and prescribed local, state, and federal regulations. - Completion of an accredited bachelor’s degree in a clinical field.
- Professional licensure or registration in a clinical field is required.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
- Two years in a supervisory position preferred.
- Knowledge and ability to apply community health principles and practices.
- Ability to deal effectively with high levels of stress.
Responsibilities
- Ensures that upon admission to Lorian all patients are thoroughly assessed and that based on that assessment the appropriate clinical services are rendered to the patient using Lorian’s interdisciplinary team approach.
- Evaluates clinical practices of specified disciplines through the review of clinical records to ensure that standards of care are met, and quality services are provided in accordance with prescribed Lorian’s, local, State, and Federal guidelines
- Keeps informed on the latest updates in state and federal regulations, community standards, and stays abreast of the latest developments in the field of nursing while actively promoting timely dissemination of all developments and changes to the clinical staff.
