Northside has been a staple of the Harlem community since its founding in 1946 and a pioneer at the intersection of education and behavioral health for Black and Latino children. Several of its programs were the first in Harlem and established the standard of care for many social service agencies. Northside’s founders, Doctors Kenneth, and Mamie Clark were psychologists whose breakthrough Black/White Doll Study was used as evidence in Brown v. Board of Education, which declared public school segregation unconstitutional.

ABOUT THE ROLE:
Reporting to the Assistant Clinical Director, the Registered Nurse (RN) provides high-quality, compassionate, and evidence-based nursing care to patients under the supervision of the Assistant Clinical Director. The Registered Nurse will play a critical role in promoting patient wellness, managing acute and chronic conditions, and ensuring compliance with clinical protocols and regulatory standards. This position aims to support interdisciplinary collaboration, enhance patient outcomes, and uphold the mission and values of the healthcare organization.
DUTIES AND
RESPONSIBILITIES:
Conduct initial and periodic health assessments to identify medical needs and risk factors.
Perform routine physicals to monitor overall health and detect early signs of illness.
Track and manage chronic conditions and comorbidities, including Tobacco Use Disorder.
Facilitate group sessions focused on health education, wellness strategies, and chronic disease management.
Provide individualized treatment plans for clients with Tobacco Use Disorder.
Lead group-based interventions to support smoking cessation and promote healthier lifestyles.
Schedule medical appointments, order lab tests, initiate referrals, and manage follow-up care to ensure continuity and quality of services.
Measure and record vital signs (e.g., blood pressure, temperature, weight) during clinic visits and outreach activities.
Maintain accurate and timely records in the Electronic Health Record (EHR) system and collaborate with psychiatric providers and clinicians to support integrated care. Provide reminders, health education, and follow-up to encourage adherence to treatment plans and improve health outcomes.

Ability to perform thorough health screenings and physical exams, identify comorbidities, and recognize early signs of illness or complications.
Skilled in organizing patient care activities, including scheduling appointments, managing referrals, and ensuring timely follow-up to support continuity of care.
Proficient in educating patients on health conditions, treatment plans, and lifestyle changes, especially in areas like smoking cessation and chronic disease management.
Accurate and efficient in documenting patient information, clinical notes, and communication with other providers using EHR systems.
Capable of leading health monitoring and smoking cessation groups, fostering engagement, and promoting peer support among patients.
Strong communication skills to collaborate effectively with psychiatric providers, clinicians, and other healthcare professionals to ensure integrated care.

QUALIFICATIONS:
Bachelor of Science in Nursing (BSN) from an accredited nursing program.
Current and valid Registered Nurse (RN) license in the state of New York (or applicable state of practice).
Tobacco Treatment Specialist (TTS) certification (preferred for roles involving smoking cessation support).
Minimum of 2–4 years of clinical nursing experience, preferably in behavioral health, primary care, or community health settings.
Experience with health monitoring, chronic disease management, and patient education is highly desirable.
Proficiency in using Electronic Health Record (EHR) systems for documentation and care coordination.
Familiarity with medical equipment used for routine health monitoring (e.g., blood pressure cuffs, thermometers, scales).
Strong communication and interpersonal skills.
Ability to work collaboratively in interdisciplinary teams.
