As a Manager, Care Coordination, you will oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions. Every day you will manage and mentor Care Coordination RNs, monitor performance, ensure regulatory compliance, and collaborate with interdisciplinary teams to optimize patient flow and effectiveness.
