- Conduct comprehensive patient assessments to determine individual care needs and develop tailored care plans.
- Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and medical protocols.
- Manage complex medical procedures including wound suturing, tube feeding, ventilator management, and dialysis support.
- Monitor vital signs, perform physical examinations, and evaluate patient responses to treatments using advanced physiology knowledge.
- Utilize electronic health records (EHR/EHR systems) such as Alora Plus for accurate documentation of patient care activities and medical records management.
- Coordinate with interdisciplinary teams for discharge planning, case management, and ensuring compliance with HIPAA and NCQA standards.
- Provide specialized care for patients with developmental disabilities, memory care needs, or those requiring hospice & palliative medicine support.
- Deliver emergency response procedures including acute pain management, trauma medicine, and critical & intensive care interventions when necessary.
*Experience*
- Valid Georgia LPN or RN license with current state certification required; experience working with individuals with disabilities highly preferred.
- Proven experience in home care settings or long-term care environments such as nursing homes or senior living communities.
- Demonstrated proficiency in critical care areas including ICU or trauma center experience; familiarity with Level I & II trauma center protocols is advantageous.
- Skilled in handling complex medical procedures like cardiology-related interventions (e.g., cardiac catheterization), ostomy care, or gastrostomy tube management.
- Strong background in managing infectious disease cases, infection control practices, and laboratory specimen collection/processing.
- Experience working with EMR/EHR systems such as Alora Plus; knowledge of ICD coding (ICD-9/ICD-10) is beneficial.
- Excellent patient assessment skills combined with the ability to triage effectively in urgent situations across outpatient or inpatient settings.
- Knowledge of Workers’ Compensation law related to occupational health cases; familiarity with pain management techniques and behavioral health considerations is a plus.
