Coordinate multidisciplinary teams to ensure a seamless, holistic care experience where every specialist aligns with the patient's goals. Monitor and interpret patient conditions in real-time, acting as the critical link between the patient and medical staff to trigger timely interventions.

Advocate for patients and families by managing the discharge process, ensuring a smooth and confident transition from hospital to home. Licensure / Certification / Registration Required Credential(s): Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date. One or more of the following: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date.

American Heart Association or American Red Cross accepted. Instructor specializing in Basic Life Support obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
