Roze Room is a leading provider of Hospice and Palliative Care, celebrating 25 years of service to Southern California communities. Full-Time Hospice RN Case Manager (RNCM) – Field Based. As a Hospice RN Case Manager, you'll guide patients and families through one of life's most vulnerable transitions, delivering comfort, dignity, and compassionate care where it matters most, at home. Lead and coordinate individualized hospice care plans. Provide skilled nursing care including pain and symptom management. Supervise LVNs and CHHAs in the field. Collaborate with physicians and the interdisciplinary team. Educate and support patients and families during critical moments.
