Overview
We are seeking a compassionate and dedicated Home Health Nurse to join our team. The ideal candidate will provide essential assistance to patients in their homes, ensuring their comfort and well-being. At ALLHEART HHC we look for nurses who are compassionate, adaptable, detail oriented and will serve as efficient advocates for our patients.
Duties
- Assess and document patient condition at each visit.
- Complete evaluation of patient safety, functional status, medications, etc,.
- Schedule and attend patient appointments.
- Communicate with interprofessional team and patient.
- Monitor patient condition(s) and report any changes to PCP.
Requirements
- Experience in related field is preferred.
- Strong interpersonal and interprofessional skills.
- Ability to act quickly, safely and efficiently in emergencies.
- Flexibility and ability to work in a variety of environments.
- Punctuality and efficient time management.
- Valid Drivers license, ability to pass federal background check and willingness to travel within surrounding area.
