What You'll Do

- Monitor and assess the health status of individuals served and maintain individualized healthcare plans.
- Administer nursing and medical services in compliance with Delaware nursing standards and agency policies.
- Coordinate medical appointments, hospital follow-up care, medication management, and emergency response services.
- Provide staff education and training related to medical and nursing care needs.
- Maintain accurate clinical documentation, medical records, and confidentiality in accordance with HIPAA standards.
- Collaborate with interdisciplinary teams, healthcare providers, and program staff to ensure quality care delivery.
- Provide on-call support, backup nursing coverage, and additional assistance as needed.
What You Bring
- Active Registered Nurse (RN) license in the State of Delaware required.
- Minimum two years of nursing experience preferred, ideally supporting individuals with cognitive or developmental disabilities.
- Strong communication, organizational, and problem-solving skills required.
- Valid driver’s license and ability to meet physical job requirements, including lifting up to 50 pounds.
