About this role

Registered Nurse / Nurse Case Manager position servicing patients in the Macon, GA area. Visits patients in homes and facilities (ALF and ILF's), facilitates with appropriate providers, initiates telemedicine visits, completes care management plans, AWV's, cognitive impairment screenings, SDoH assessments, medication reconciliation, and post-discharge education. Full-time, salary-based, 8-hour shifts Monday-Friday (8am-5pm). Travel required daily with reliable transportation.
