About this role
As a Manager, Care Coordination, you will oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions. Every day you will manage and mentor Care Coordination RNs, monitor performance, ensure regulatory compliance, and collaborate with interdisciplinary teams to optimize patient flow and effectiveness. To be successful, you will demonstrate strong leadership, strategic planning, and clinical expertise, driving continuous improvement and optimal patient care coordination for organizational goals.
