Position Overview The LPN Care Coordinator provides direct nursing care to residents while overseeing the day-to-day clinical activities of nurses and nursing assistants. This role combines leadership and hands-on nursing practice, ensuring care delivery meets the highest standards of quality and compliance. The Care Coordinator works collaboratively with the Director of Nursing (DON) and Administrator to support resident outcomes and staff development. Key Responsibilities Provide and coordinate direct resident care in accordance with care plans and physician orders. Assess and monitor residents’ physical and emotional conditions, reporting changes promptly. Supervise and support nursing staff in daily care delivery. Ensure accurate and timely documentation of resident assessments, treatments, and medications. Safely administer and monitor medications and treatments. Serve as a role model for professional conduct and adherence to care center policies. Participate in quality improvement initiatives, training, and care planning processes. Act as Nurse in Charge when delegated, ensuring regulatory and safety compliance. Qualifications Current, unencumbered LPN license in the State of Minnesota. Current CPR certification. Strong leadership, organization, and communication skills. Ability to provide compassionate, resident-centered care. Skilled nursing or long-term care experience preferred.
