Premium Care MIHP LLC is seeking a compassionate and motivated Registered Nurse (RN) to join their growing team as an independent contractor providing services to pregnant individuals, infants, and families enrolled in Michigan's Maternal Infant Health Program (MIHP). Responsibilities include conducting maternal and infant risk assessments, providing health education and support to families, completing home visits and virtual visits, coordinating care with healthcare providers and community resources, developing individualized care plans, documenting services within the EMR, participating in interdisciplinary collaboration with social work staff, and maintaining compliance with MIHP guidelines.

Requirements

  • Active Michigan Registered Nurse license in good standing
  • Strong communication and organizational skills
  • Comfortable working independently
  • Reliable transportation for home visits within Wayne and Oakland Counties
  • Computer proficiency and ability to complete electronic documentation
  • Computer literacy
