What You'll Do
- Respond to after-hours hospice calls and urgent patient needs.
- Perform emergency nursing visits for symptom management, pain control, medication administration, and clinical assessment.
- Provide telephone support and education to patients, families, and caregivers.
- Collaborate with physicians and the interdisciplinary hospice team to coordinate care.
- Complete accurate and timely EMR documentation.
What You'll Bring
Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred; nurses from home health, oncology, ICU, palliative care, or other end-of-life care settings are encouraged to apply.
- Excellent assessment and critical thinking skills.
- Ability to work independently while managing urgent clinical situations.
- Strong communication and compassionate patient care.
- Availability to participate in the 7 On / 7 Off evening and overnight schedule.
