Agape Hospice & Palliative Care is seeking an experienced and compassionate *Per Diem Admissions Registered Nurse (RN)* to complete hospice admissions throughout Los Angeles and Orange County.

As a clinician-owned, Joint Commission-accredited hospice, we are committed to providing exceptional end-of-life care with dignity, compassion, and respect. This position is ideal for an RN who enjoys working independently, excels in patient and family education, and is passionate about guiding patients through the hospice admission process.

*About the Role*

As an Admissions RN, you will be responsible for conducting hospice admission visits, completing comprehensive nursing assessments, educating patients and families about hospice services, and coordinating a smooth transition into hospice care.

Working closely with physicians, intake staff, and the interdisciplinary team, you will ensure each patient receives timely, compassionate, and compliant admission services.

*Responsibilities*

  • Complete hospice admission and start-of-care visits in patients' homes, assisted living communities, and skilled nursing facilities.
  • Perform comprehensive nursing assessments to determine hospice eligibility.
  • Review physician orders, medical history, medications, and clinical documentation.
  • Educate patients and families about hospice philosophy, services, benefits, and the plan of care.
  • Complete all admission paperwork, consents, and required documentation.
  • Develop the initial plan of care in collaboration with the hospice physician and interdisciplinary team.
  • Coordinate admission needs with intake, case managers, and clinical leadership.
  • Identify immediate symptom management needs and initiate appropriate interventions.
  • Accurately document all clinical findings in the agency's electronic medical record (EMR).
  • Ensure compliance with Medicare Conditions of Participation and agency policies.
  • Provide compassionate support to patients and families during the transition to hospice services.

Compensation

  • *Hospice Admissions:* *$300 per completed admission*
  • *PRN visits: $100 per completed visit *(if offered)
  • *Hourly Rate:* *$60–$63/hour* for required meetings, training, and approved hourly tasks
  • Mileage reimbursement provided

*Per Diem Schedule*

This is a true per diem position with admissions assigned based on referral volume and operational needs. Weekend availability is highly preferred. As patient census grows, additional admission opportunities may become available.

*Qualifications*

  • Current California Registered Nurse (RN) license in good standing.
  • Minimum of one year of RN experience required.
  • Previous hospice admissions or hospice nursing experience strongly preferred.
  • Excellent assessment, critical thinking, and clinical decision-making skills.
  • Strong communication, education, and documentation abilities.
  • Ability to work independently while collaborating with the interdisciplinary team.
  • Reliable transportation, valid driver's license, and current auto insurance required.
  • Bilingual Spanish preferred.
