We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will help ensure patients receive personalized treatment, comfort, and support while adhering to all safety and confidentiality standards.   • Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs.
  • Administer medications and injections accurately, including IV infusions and catheterizations, following prescribed protocols.
  • Assist with medical procedures such as sterile dressing changes, suturing support, specimen collection, and tube feeding.
  • Manage airway care and ventilator support for patients requiring respiratory assistance.
  • Document all patient interactions thoroughly using Electronic Health Records (EHR) systems while maintaining strict compliance with HIPAA and clinical confidentiality policies.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs.
  • Provide education and support to patients and their families on health management, medication administration, and post-discharge care instructions.

*Requirements*

  • Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals (including ICU or trauma centers), assisted living communities, or home health environments.
  • Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and documentation standards.
  • Hands-on experience with EMR/EHR systems like Epic or Cerner is highly preferred.
  • Competency in performing clinical skills such as venipuncture, phlebotomy, first aid, CPR/BLS certification, and sterile techniques.
  • Critical care experience or specialized knowledge in dementia care, Alzheimer's care, memory care, hospice support, pain management, infectious disease care, or pediatric nursing is advantageous.
  • Ability to work effectively in fast-paced environments requiring triage skills and acute care management.
  • Excellent communication skills for caregiver interaction and client education.
  • Knowledge of long-term care regulations and geriatric patient needs is desirable.
