• Per Visit Rates (Varies on Visit Type)
• On-Call Rates:

• Week Day: $2 per hour (Plus hourly and/or visit rate if called)
• Monday 5:00 pm – Tuesday 8:00 am
• Tuesday 5:00 pm - Wednesday 8:00 am
• Wednesday 5:00 pm – Thursday 8:00 am
• Thursday 5:00 pm – Friday 8:00 am

• Weekend: $3 per hour (Plus hourly and/or visit rate if called)
• Friday 5:00 pm – Monday 8:00 am

• Holiday Rate: $5 per hour (Plus hourly and/or visit rate if called)

*Qualification*

• *Valid RN License: *Current and unrestricted RN license in the state of practice

Experience

*2 years of RN experience, 1 year of home health experience (required),

• *OASIS Assessment:* Working knowledge of the OASIS Assessment (required)

• *Clinical

*Proficient clinical skills and knowledge of home health practices, including assessments, wound care, medication management, and disease management.

*5. Case Management Expertise: *Strong understanding of case management principles, including care coordination, admission, discharge planning, and utilization review.

*6. Documentation and Charting: *Ability to accurately document patient assessments, interventions, and outcomes in compliance with agency policies and regulations.
