Full Time Hospice RN Case Manager (RNCM) providing hands-on hospice nursing care, coordinating services, and guiding patients and families through every step of their hospice journey.
Schedule
Monday - Friday, 8 am - 4:30 pm. Territory: Rhea and Bledsoe County. Responsibilities include responding to clinical referral information, partnering with Clinical Director to assess client eligibility, delivering direct patient care, applying nursing skills, educating patients and families, and sharing call duties.
