Registered Nurse (RN) – Home Health | PRN. Home Sweet Home Health Agency provides skilled, compassionate healthcare services that help patients recover, maintain independence, and live safely in the comfort of their own homes. Seeking compassionate and skilled Registered Nurses (RNs) to join the growing Home Health team in Maricopa County, AZ.
Responsibilities include conducting comprehensive nursing assessments in patients' homes, delivering skilled nursing care including wound care, medication management, treatments, and patient monitoring, developing and updating individualized home health care plans, educating patients and caregivers, collaborating with physicians and interdisciplinary care teams, completing clinical documentation, and monitoring patient progress.

Requirements
- Current Arizona Registered Nurse (RN) license in good standing
- Graduate of an accredited nursing program (BSN preferred)
- Minimum of 1 year of RN experience required
- Home Health nursing experience strongly preferred
- Experience working with geriatric patient populations preferred
- Valid driver's license, reliable transportation, and ability to travel
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational abilities
