Travel Licensed Practical Nurse Long Term Care Job

• Setting: skilled nursing facility, long-term care, travel assignment

Responsibilities

• Provide direct patient care to residents under supervision of a registered nurse, in a long-term care setting
• Administer medications and perform treatments as ordered; monitor and document patient responses
• Conduct routine nursing assessments, identify changes in condition, and communicate findings to the care team
• Assist residents with activities of daily living, mobility, and personal care while promoting safety and dignity
• Collaborate with the interdisciplinary team to update care plans and ensure continuity of care
• Adhere to infection control practices, safety protocols, and documentation standards

Requirements

• Active Licensed Practical Nurse license in Vermont or eligibility for licensure in Vermont
• Graduate of an accredited practical nursing program
• Current Basic Life Support (BLS) certification or equivalent
• 1 year of nursing experience in a long-term care or skilled nursing setting preferred
• Ability to work in a travel assignment and in a skilled nursing facility environment
