Duties and Responsibilities Provide skilled nursing care under the direct supervision of a Registered Nurse Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventive nursing care Be responsible and accountable for decisions based on the individual’s nursing experience and educational preparation Assist the Registered Nurse in carrying out the plan of care Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician orders Assist with the preparation, implementation, and ongoing evaluation of the patient care plan Recognize and understand the effects of social and economic factors on patients and provide for their emotional and physical comfort and safety Teach patients appropriate self-care techniques Foster cooperative teamwork by understanding the functions of other care team members and actively participating in team and staff conferences Assume responsibility for personal and professional growth through participation in nursing organizations, in-service programs, and on-the-job learning Prepare clinical and progress notes (excluding admissions) Observe, record, and report patient symptoms, reactions, and changes in condition to the appropriate personnel, including: General physical and mental condition Signs and symptoms indicative of changes in condition Psychosocial stressors affecting patients and families Assist with patient rehabilitation in accordance with the medical care plan by encouraging independence and use of individual capabilities Report abnormalities in the patient’s range of motion, body mechanics, or body alignment Reliably report to assigned patient locations

Job Conditions Ability to communicate effectively in English, both verbally and in writing Ability to access patients’ homes, which may not be routinely wheelchair accessible Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and patient care Physical activities include walking, sitting, stooping, standing, lifting, and turning patients

Work Environment Contact with clients under a wide variety of circumstances Care provided in clients’ living environments, including private homes, independent or assisted living settings, hospitals, or long-term care facilities May be required to respond to emergency situations Position includes tasks with potential exposure to blood, body fluids, tissues, and other potentially infectious materials

Qualifications Graduate of an accredited school of Practical Nursing Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Kansas Home health or hospice experience preferred (training provided for the right candidate) Knowledge of basic nursing principles and patient care practices Ability to pass a criminal background check Current CPR certification through an approved provider (in-person skills verification required) Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position, in accordance with applicable law.
