In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will ensure that patients receive personalized, safe, and effective treatment while fostering a supportive environment that promotes healing and well-being.

This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations, including geriatric individuals, those with chronic conditions, and patients requiring acute or emergency care.

Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately, including IV infusions and catheter care, following strict protocols to ensure patient safety.
  • Manage complex care needs such as Alzheimer's disease, dementia, memory care, hospice support, and acute pain management for diverse populations.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
  • Conduct diagnostic evaluations like specimen collection/processing, tube feeding management, and respiratory support including ventilator care.
  • Provide patient education on health coaching topics such as medication administration, discharge planning, and lifestyle modifications.
  • Maintain detailed medical documentation in compliance with HIPAA confidentiality policies and ensure accurate coding using ICD-9/ICD-10 standards for billing and reporting purposes.
  • Respond promptly to triage calls or urgent situations in emergency room or urgent care settings, utilizing your experience in trauma centers or Level I/II trauma environments.
  • Support infection control practices through aseptic techniques and adherence to infection prevention protocols across all patient interactions.
  • Assist with discharge planning and case management to ensure seamless transitions of care within skilled nursing facilities or home & community care environments.

Skills

  • Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal or neonatal ventilator patients, individuals with developmental disabilities, or those requiring specialized dementia or Alzheimer's care.
  • Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures such as venipuncture, suturing, catheterization, first aid, and airway management.
  • Proficiency in EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and medical records management.
  • Skilled in medication administration techniques including injections and IV insertions; capable of managing complex cases involving acute pain or infectious diseases.
  • Familiarity with long-term care regulations and experience in skilled nursing facilities or sub-acute care settings is highly desirable.
  • Ability to communicate effectively with patients of all ages—children to seniors—and coordinate with caregivers for optimal patient outcomes.
  • Knowledge of ICD coding standards (ICD-9/ICD-10) for diagnostic classification; experience in medical imaging or laboratory procedures is a plus.
  • Capable of working efficiently under pressure in fast-paced environments such as emergency rooms or hospital medicine units while maintaining clinical confidentiality policies.
  • Demonstrated commitment to ongoing professional development through certifications like Basic Life Support (BLS), CPR, sterile processing techniques or specialized trauma center training is advantageous.
