Nina's Home healthcare is hiring LPNS and RNs for a SN visit case in Groveport. The nurse will complete two visits a week for a patient who needs help with taking her glucose and INR.

  • Deliver comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately following physician orders, including IV infusions and medication management for pain, infectious diseases, or chronic conditions.
  • Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
  • Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care residents, or individuals requiring hospice or palliative care.
  • Manage airway and ventilator support, perform catheterization and sterile procedures while maintaining aseptic techniques.
  • Support emergency response efforts by performing first aid, CPR certification maintenance, and triage assessments in urgent or acute care situations.
  • Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks while ensuring compliance with HIPAA and confidentiality policies.
  • Collaborate with multidisciplinary teams to develop discharge plans, case management strategies, and long-term care plans aligned with regulatory standards such as NCQA or long-term care regulations.
  • Communicate effectively with patients’ families and caregivers to provide education on health coaching, medication administration, nutrition, and post-discharge instructions.

*Experience*

  • Valid Licensed Practical Nurse license in good standing with relevant state licensing boards.
  • Proven experience working in diverse healthcare environments including hospitals (Level I Trauma Center or Level II Trauma Center experience preferred), nursing homes, skilled nursing facilities, outpatient clinics, or home & community care settings.
  • Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; experience with medical documentation and ICD coding (ICD-9/ICD-10).
  • Critical care experience including ICU or emergency room exposure highly desirable; experience working with trauma patients or in acute care settings is a plus.
  • Familiarity with specialized areas such as geriatrics, dementia/Alzheimer’s care, pediatric or neonatal ventilator management is advantageous.
  • Knowledge of infection control practices and sterile technique; experience with IV insertion/insertion techniques including IV infusion therapy is essential.
  • Ability to perform physical examinations accurately and assess patient physiology effectively; experience working with individuals with disabilities or developmental delays is beneficial.
  • Strong communication skills for effective caregiver interaction and client education; ability to work independently while collaborating within a multidisciplinary team environment.
