What You'll Do

  • Deliver skilled nursing care within an individualized hospice plan of care
  • Assess patients' physical, psychological, social, and environmental needs and adjust care accordingly
  • Include patients and families as true partners in the care planning process
  • Coordinate seamlessly across the care team to ensure nothing falls through the cracks
  • Provide exceptional, compassionate service to patients, families, and referral sources
  • Support fellow team members — because great hospice care is a team effort

What You'll Need

  • Active Colorado RN license in good standing
  • Current CPR certification
  • Reliable, insured transportation
  • Prior hospice experience preferred — or at least 1 year of recent med/surg experience
  • Strong communication, critical thinking, and organizational skills
  • A genuine calling for end-of-life care and supporting families through life's hardest moments
