• Perform thorough patient assessments, including vital signs, physical examinations, and comprehensive health histories to develop individualized care plans.
• Administer medications via various routes such as injections, IV infusions, and catheter care, ensuring strict adherence to aseptic techniques and medication protocols.
• Manage complex wound care procedures including suturing, ostomy care, and dressing changes while maintaining infection control standards.
• Conduct specialized treatments such as botulinum toxin injections for therapeutic purposes in ophthalmology or neurological conditions.
• Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks to document patient interactions accurately and efficiently.
• Provide education on disease management, medication administration, nutrition, and self-care strategies tailored to each patient's condition.
• Coordinate with multidisciplinary teams for discharge planning, case management, and long-term care strategies across settings including hospice, memory care, and assisted living facilities.
• Respond promptly to triage calls and urgent health concerns with critical thinking skills honed through experience in emergency medicine or ICU settings.
• Assist with diagnostic evaluations such as laboratory specimen collection or imaging procedures like sonography when necessary.
• Support patients with complex needs such as dialysis management, ventilator care, or trauma medicine in accordance with best practices and NCQA standards.

*Requirements*

• Valid RN or LPN license in good standing; additional certifications such as BLS (Basic Life Support) are required.
• Demonstrated experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities, or those requiring memory care.
• Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or similar platforms for medical documentation.
• Strong knowledge of medical terminology, anatomy & physiology, ICD coding (ICD-9/10), and infection control protocols.
• Critical care experience preferred—such as ICU or trauma center background—and familiarity with emergency response procedures.
• Ability to perform complex procedures including IV insertion, phlebotomy, tube feeding management, and aseptic technique adherence.
• Excellent communication skills for patient education and interdisciplinary collaboration; experience in home & community-based care is a plus.
• Flexibility to adapt to various clinical environments including hospital medicine, outpatient clinics, urgent care centers or sub-acute settings.
