About this role
*Opportunity Summary*
As our *Nursing Case Manager*, you will deliver comprehensive education, hands-on skills training, and coordinated care services to promote healthcare compliance and improve outcomes for vulnerable families and children. Full-time availability required.
*Key Responsibilities*
Every day, you will manage your schedule, engage with clients, provide client and caregiver education, demonstrate and reinforce clinical skills, assess home environment and barriers to care, monitor compliance and health status, coordinate care, and communicate with multidisciplinary team.

*Qualifications*
- Unencumbered Indiana nursing license
- Minimum one (1) year of patient care experience as a nurse, including long-term care
- CPR/First Aid certification
- Valid driver’s license and auto insurance
- Strong communication and time-management skills
- Strong computer skills
