Summary
Essential Duties and Responsibilities
The Health and Wellness Director provides clinical leadership for the community's Health Services department and is responsible for ensuring residents receive quality, person-centered care in accordance with Colorado Assisted Living Residence (ALR) regulations, accepted standards of nursing practice, and company policies. The Health and Wellness Director partners with the Executive Director and interdisciplinary team to promote resident well-being, regulatory compliance, and operational excellence.
ESSENTIAL DUTIES AND RESPONSIBILITES include the following. Other duties may be assigned.
Resident Assessment and Care Planning
- Oversees the resident assessment process to ensure comprehensive assessments are completed prior to admission, upon significant change in condition, and at intervals required by state regulations.
- Develops, implements, reviews, and updates individualized service plans that reflect each resident's assessed needs, preferences, abilities, and physician recommendations.
- Coordinates and facilitates resident care conferences with residents, families, physicians, and the interdisciplinary team as appropriate.
- Monitors residents for changes in physical, cognitive, behavioral, and psychosocial status and ensures timely reassessment, documentation, and revision of service plans.
- Supports residents and families during transitions in care, including admissions, changes in condition, hospitalizations, hospice services, and discharge planning.
- Ensures residents receive services consistent with their individualized service plans while promoting dignity, independence, choice, and quality of life.
Clinical Oversight
- Provides clinical oversight of resident care services to ensure compliance with physician orders, company policies, accepted standards of practice, and Colorado ALR regulations.
- Monitors resident health status, including weights, vital signs, skin integrity, nutrition, hydration, and other clinical indicators, ensuring appropriate interventions are initiated and documented.
- Coordinates ancillary healthcare services including home health, hospice, therapy, laboratory services, pharmacy, and specialty providers.
- Ensures physician orders are obtained, implemented, documented, and communicated to appropriate team members in a timely manner.
- Reviews resident incidents, accidents, falls, and changes in condition to ensure appropriate assessment, intervention, follows-up, documentation, and required notifications.
- Notifies the Executive Director, physician, resident representative, and other appropriate parties of significant changes in condition, emergencies, or reportable incidents in accordance with company policy and regulatory requirements.
- Ensures effective clinical communication during shift change, interdisciplinary meetings, and transitions of care.
- Staffing Coverage: Provides direct resident care and/or assumes appropriate staffing responsibilities when necessary to maintain safe and adequate coverage, including filling an open shift on short notice when other qualified staff are unavailable to cover the vacancy. Supports continuity of care and ensures resident needs and regulatory staffing requirements are maintained.
- 24/7 On-Call Responsibility: Serves as a clinical and operational resource for urgent resident care needs, health-related concerns, staffing issues, emergencies, and other matters requiring immediate leadership attention. Responds promptly to on-call communications, provides direction to staff, coordinates appropriate interventions and notifications, and escalates significant concerns to the Executive Director and/or appropriate healthcare professionals, as necessary.
Medication Management
- Oversees the community's medication management program to ensure medications are ordered, received, stored, administered, documented, and disposed of in accordance with Colorado regulations and company policy.
- Ensures Medication Administration Records (MARs), physician orders, and medication documentation remain accurate and current.
- Provides training, competency validation, supervision, and ongoing oversight for Medication Administration Personnel in accordance with Colorado regulations.
- Conducts routine medication audits and monitors pharmacy performance to ensure compliance, accuracy, and resident safety.
- Ensures consultant pharmacist reviews are completed as required and that recommendations are reviewed and implemented as appropriate.
Regulatory Compliance and Quality Assurance
- Ensures the community maintains compliance with all applicable Colorado Assisted Living Residence regulations, licensing requirements, and company standards.
- Monitors resident eligibility for admission and continued residency, ensuring the community provides services within its licensed scope of care.
- Maintains resident clinical records that are accurate, complete, confidential, and compliant with state regulations and company policy.
- Ensures emergency contact information, advance directives, physician information, and other required documentation remain current.
- Oversees infection prevention and control practices, including surveillance, education, outbreak response, and required reporting in consultation with the Clinical Director and public health authorities.
- Participates in quality assurance and performance improvement activities, including audits, trend analysis, corrective action plans, and survey readiness.
- Participates in state surveys, investigations, and regulatory inspections, implementing corrective actions as necessary.
Leadership and Team Development
- Recruits, hires, orients, coaches, evaluates, develops, and retains qualified Health Services team members in partnership with the Executive Director.
- Ensures team members complete required orientation, annual education, competency validation, certifications, and ongoing training as required by Colorado regulations and company policy.
- Evaluates performance and provides ongoing coaching to support professional growth and accountability.
- Prepares and maintains staffing schedules that support resident care needs while managing labor resources effectively.
- Reviews and approves staff time-off requests and maintains appropriate staffing coverage.
- Trains staff in documentation standards, resident rights, infection prevention, emergency procedures, and electronic medical record (EMR) processes.
- Promotes teamwork, accountability, professionalism, and exceptional customer service.
- Ensures resident rights, dignity, privacy, informed choice, and self-determination are promoted and protected in accordance with state and federal requirements.
- Collaborates with licensed healthcare professionals and ensures delegated nursing tasks are performed in accordance with the Colorado Nurse Practice Act and applicable regulations.
- Conducts routine observation and competency validation of caregivers and QMAPs to ensure safe and effective resident care.
- Maintains the Health Services department in a continuous state of survey readiness through routine audits, staff education, and quality improvement initiatives.
Financial and Operational Management
- Partners with the Executive Director regarding changes in resident service levels that may impact care needs, staffing, or resident charges.
- Manages departmental labor and operating expenses within approved budget parameters.
- Supports occupancy initiatives by participating in resident assessments, move-in planning, and family education regarding care needs.
- Maintains inventory and oversight of clinical supplies, medications, and medical equipment.
Safety and Emergency Preparedness
- Promotes a culture of resident and employee safety through education, coaching, and compliance monitoring.
- Ensures Health Services staff understand and follow emergency preparedness plans, disaster procedures, and evacuation protocols.
- Responds to emergencies and provides clinical direction, assessment, and first aid when appropriate.
- Participates in the Manager-on-Duty rotation and remains available for after-hours clinical consultation and emergencies as required.

Education or
Experience
Must be a graduate of an approved LPN or RN program and have at least one-year related experience. Current, unencumbered license to practice as an LPN or RN in the state of Colorado. Current Basic Life Support (BLS/CPR) certification.
Language Ability: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to read, write, and speak effectively in English. Uses tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations.
Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer
Skills
To perform this job successfully, an individual should have knowledge of MS Office Suite and the ability to implement and manage Electronic Medical Record software (PCC). Ability to learn new technology, as needed.
Supervisory
Responsibilities
Supervises licensed nurses, Qualified Medication Administration Personnel (QMAPs), caregivers, and other Health Services staff. In the absence of the Executive Director, supervises entire team at this location.
