Licensed Practical Nurse(LPN) – Youth Program

Schedule

• Saturday & Sunday, 8:00 AM–12:00 AM
• *Position Type:* Full-Time
• *Start:* Immediate / As soon as credentialed

*Job Responsibilities*

As an LPN, you will provide safe, compassionate, and high-quality nursing care in accordance with accepted nursing standards.

*Responsibilities include:*

• Provide patient/resident care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments in accordance with the plan of care and facility policies.
• Monitor residents' conditions and promptly report changes to the supervising nurse or physician.
• Respond appropriately to residents' clinical and healthcare needs.
• Collaborate with DSPs, nurses, program staff, families/guardians, physicians, and other members of the interdisciplinary team.
• Follow all applicable DDA, healthcare, medication-management, safety, and agency policies and procedures.
• Maintain accurate and timely nursing documentation in accordance with physician orders, care plans, and regulatory standards.
• Promote a safe, respectful, and therapeutic environment for residents and their families.
• Work collaboratively with the healthcare and interdisciplinary team to support quality resident care.
• Ethical and Professional.
• Ensures Patient Safety and Quality Care, Builds Patient Trust, Fosters Effective Collaboration with the Team, Upholds Ethical Standards, and Drives Patient Advocacy. Pay close attention to detail, follow evidence-based standards, and remain deeply accountable for their actions to prevent errors.

*Qualifications*

• *Active compact LPN license.*
• *Valid BLS/CPR certification required.*
• Ability to provide care for and manage multiple patients/residents in a fast-paced healthcare environment.
• Strong clinical judgment and decision-making skills.
• Excellent communication and interpersonal skills.
• Strong attention to detail and commitment to accurate documentation.
• Compassionate, dependable, and professional approach to resident care.
