LPN- Pediatric Home Health Nurse

Specialty Care Pediatrics is looking for nurses who are compassionate and reliable. We provide continuous in-home care to medically fragile patients needed to keep them safe with their loved ones. We match our nurses with our patients based on skill, acuity level, location, age and health condition. Our nurses make a positive and long-lasting impact on their patient’s lives by providing one-on-one care. We provide flexible shifts ranging from 4 to

Duties and Responsibilities

Implements physicians' prescribed plan of care, including assessments, Administration of medications, treatments, and therapies. Maintain ongoing tracking and appropriate documentation on patients in accordance with Specialty Care Pediatrics policies and procedures in a timely, accurate, and consistent manner. Provides health care/management, education and support of patient and/or family members regarding diagnosis, plan of care, preventative and maintenance of care.

Maintains clinical expertise and competency as well as demonstrates thorough knowledge of procedures to deliver quality primary health care in coordination with the health care team. Identifies, reports and responds promptly to changes in patient’s condition according to Specialty Care Pediatrics policy. Willing to travel to any clinic location as needed.

Proficiency in basic nursing skills and knowledge of the Oklahoma Nursing Practice Act Knowledge of principles of health promotion, prevention and motivation Knowledge of medical terminology and appropriate abbreviations Effective verbal and written communication skills Organized and able to manage priorities Resourcefulness in problem solving Able to take and follow through with delegated tasks and accountability, and able to accept supervision and direction, teamwork orientation
