Hospice case Manager- RN PRN

• Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards.
• Coordinate interdisciplinary team efforts to deliver holistic patient care, including symptom management and emotional support.
• Manage patient intake processes, including reviewing medical records, vital signs, and conducting diagnostic evaluations.
• Oversee discharge planning and post-acute care arrangements to ensure smooth transitions between inpatient, outpatient, or home settings.
• Perform regular patient visits to monitor condition changes, update care plans, and provide education to patients and families on disease management.
• Maintain accurate documentation of medical records, case management activities, and compliance with HIPAA regulations.
• Collaborate with hospital staff, primary care providers, and community resources to optimize patient outcomes across various healthcare settings such as ICU, ER, Geriatrics, Pediatrics, or Behavioral health units.

*Skills*

• Extensive critical care experience including ICU, emergency room (ER), or trauma center settings such as Level I or Level II Trauma Centers.
• Proven expertise in hospital medicine, medical-surgical units, or post-acute care environments with a focus on acute care management.
• Strong knowledge of physiology, infectious disease care, and diagnostic evaluation techniques essential for complex patient assessments.
• Experience in managed care programs and utilization management to coordinate appropriate services efficiently.
• Skilled in discharge planning, medical terminology interpretation, and managing medical records within inpatient or outpatient clinics.
• Ability to perform comprehensive patient assessments involving vital signs monitoring and case management strategies for diverse populations including geriatrics and pediatrics.
• Familiarity with HIPAA compliance standards and medical-surgical nursing practices to ensure privacy and quality of care delivery.
