*Job Description:*

We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our team on a part-time basis. The provider will conduct in-home visits, delivering comprehensive medical care to a diverse patient population.

Responsibilities

*

Perform in-home patient evaluations and physical exams

Diagnose and manage acute and chronic conditions

Prescribe medications and treatments as appropriate

Document visits accurately in the electronic medical record

Coordinate care with physicians, nurses, and other healthcare professionals

Provide patient and family education

Requirements

*

Active and unrestricted NP or PA license in New York State

Board certification (NP or PA)

DEA registration (or ability to obtain)

Prior experience in primary care, geriatrics, or home-based care preferred

Strong clinical and communication skills

Reliable transportation and willingness to travel within Suffolk County
