About this role
The Admissions Nurse (RN) is responsible for conducting hospice admission assessments, determining hospice eligibility in collaboration with the hospice physician/medical director, and initiating hospice services in accordance with physician orders, hospice regulations, and organizational policies. This role serves as a key clinical liaison among patients, families, referral sources, physicians, and the interdisciplinary group (IDG). This position will support Scottsdale.

- Conduct comprehensive hospice admission assessments, including physical, psychosocial, and symptom evaluations
- Determine hospice eligibility in collaboration with physician and/or medical director and the interdisciplinary group
- Ensures admission diagnoses and supporting documentation align with applicable Local Coverage Determinations (LCDs)
- Educate patients and families on hospice philosophy, services, and care plans.
- Obtain and verify physician orders, consents, and required admission documentation
- Coordinate timely admissions from hospitals, facilities, homes, and other referral sources
- Initiate plans of care and communicate patient needs to the interd
