Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and have the ability to work in a fast-paced environment. As a Staff Nurse (RN), you will be responsible for providing high-quality nursing care to patients, collaborating with healthcare professionals, and ensuring the delivery of effective treatment plans. This is for residents in a long-term care community as well as patients in short-term rehab therapy, enjoy the opportunity to connect with patients and their families.
Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any changes in condition to the healthcare team.
- Provide education and support to patients and their families regarding health management and treatment options.
- Assist in case management, ensuring continuity of care for patients with complex health needs.
- Collaborate with interdisciplinary teams to coordinate care for patients, including those requiring infectious disease care or ostomy care.
- Utilize electronic health record systems such as Point Click Care for documentation and patient management.
Skills
- Proficient in patient assessment techniques and clinical decision-making.
- Knowledge of coding practices relevant to nursing documentation.
- Familiarity with case management principles and practices.
- Experience in infectious disease care is preferred.
- Strong communication skills, both verbal and written, for effective patient interaction.
- Ability to work collaboratively within a multidisciplinary team environment.
- Detail-oriented with strong organizational skills to manage multiple patient cases effectively.
