LPN

What You’ll Do

• Provide skilled nursing care in the home setting under the direction of an RN
• Follow physician orders and individualized plan of care
• Monitor patient conditions and report changes to the RN and physician
• Administer treatments and perform nursing procedures per agency standards
• Educate patients and caregivers on medications, disease processes, and home care
• Document visits in compliance with Medicare guidelines
• Ensure adherence to HIPAA, infection control, and regulatory requirements

Qualifications/Educational

Requirements

Licensed Practical Nurse (LPN) from a state-approved, accredited program. Oasis Home Health Care 1 year of experience. Minimum 2 years of healthcare experience Strong communication and organizational skills Ability to work independently and manage time effectively Understanding of Medicare/Medicaid regulations (preferred) Reliable transportation for travel to patient homes Must have AHCA II Background Check: https://info.flclearinghouse.com Must have a valid Driver’s License and Auto Insurance Alzheimer and Dementia 1 year of verifiable experience Current CPR

• Follow-up assessment and documentation of observed patient's condition and interventions provided at each visit

• Complete patient evaluation tasks, including medication administration and vital signs

• Administer Physician-prescribed medications

• Wound care and maintenance of skin integrity

• Reinforce patient and family education on appropriate home health care procedures and strategies

• Coordinate with RNs, physicians, physical therapists, occupational therapists and other individuals in the patient's care plan

• · Ensure patient’s comfort while supporting independence
