About this role

Develops strong business continuity relationships to support the agency's business development goals. Demonstrates flexibility in order to address referral partners' needs. Responds to all requests for information and referrals from hospital personnel and obtains the Election of Benefits for hospice enrollment of eligible patients. Gathers clinical information that is used in eligibility determinations, utilizing the Hospice Conditions of Participation, Hospice Local Coverage Determinations (LCD).
