## As a Home Visitor RN, you will

Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients

Assess and address the health and developmental needs of women and their families

Offer counseling and instruction to help families achieve targeted health and life goals

Build and maintain strong community relationships to facilitate client referrals and resource connections

Attend weekly team meetings, case conferences, and clinical supervision sessions

Mentor and precept new hires and support student nurses during home visits as needed

Ensure compliance with agency infection control and safety policies

Maintain professionalism in adherence to organizational policies on absenteeism and appearance

## Qualifications

Associate's degree in Nursing required

2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred

Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities

Bilingual (English and Spanish-speaking) preferred

A commitment to CFHI’s mission and the ability to build trusting relationships with clients and colleagues

This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
