About this role

TRU Community Care is seeking an Afterhours Registered Nurse to conduct afterhours home visits and provide care to patients as delegated by the Phone Triage Nurse. The RN will provide nursing care in accordance with the patient's hospice plan of care, cross-train on phone triage, collaborate with the interdisciplinary team, and work with patients and families on care plans.
