Summary

Polaris Healthcare and Rehabilitation Center, a beautiful 100-bed facility located at the Milford Wellness Village, is seeking a compassionate and experienced RN Unit Manager to join our team. As a RN Unit Manager, you will be responsible for managing the unit, utilizing the nursing process to assess, plan, implement and evaluate resident care, and ensuring that our residents receive high-quality, compassionate care.

Responsibilities

Utilize the nursing process to assess, plan, implement, and evaluate resident care

Assess signs and symptoms indicating changes in the patient's condition

Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions

Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards

Revises the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions

Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities

Qualifications

Must be a licensed RN with a valid license

Familiarity with Federal, State, and local long-term care requirements

Two years of professional nursing experience
