Compassionate Keepers Home Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to conduct in-home client assessments on an as-needed basis.

This position is ideal for an LPN looking for flexible, supplemental work rather than a traditional hourly nursing schedule. The nurse will visit clients in their homes to complete required assessments, document findings, and communicate relevant information to the agency.

This role is primarily assessment and documentation focused and does not involve providing ongoing shift-based caregiving services.

Key Responsibilities

  • Conduct initial and periodic in-home client assessments as assigned.
  • Evaluate the client’s current functional status, personal care needs, mobility, and ability to complete Activities of Daily Living (ADLs).
  • Identify changes or concerns that may affect the client’s plan of care.
  • Review relevant health information, medications, diagnoses, and client-reported needs as appropriate to the assessment.
  • Complete required assessment forms and documentation accurately and within agency-established timeframes.
  • Assist with developing or updating client care plans based on assessment findings and within the LPN’s permitted scope of practice.
  • Communicate significant findings or changes in condition to agency leadership and/or the appropriate supervising clinician.
  • Observe the client’s home environment for potential safety concerns and document findings.
  • Maintain client confidentiality and comply with HIPAA requirements.
  • Follow Compassionate Keepers policies, procedures, and applicable Georgia regulations.
  • Participate in reassessments when required or when there is a significant change in a client’s condition.

Qualifications

  • Current and unrestricted Georgia LPN license.
  • Graduate of an accredited practical nursing program.
  • Current CPR/BLS certification preferred.
  • Previous home care, home health, long-term care, geriatrics, or community-based nursing experience preferred.
  • Experience completing client assessments and care-related documentation strongly preferred.
  • Strong communication and documentation skills.
  • Ability to work independently in clients’ homes.
  • Dependable transportation and willingness to travel throughout the assigned service area.
  • Ability to complete documentation electronically and meet established deadlines.
  • Must successfully complete all required background checks and agency onboarding requirements.

Schedule

This is a PRN/as-needed position. Assessments will be scheduled based on new client admissions, required reassessments, changes in client condition, and other agency needs.

The LPN will have flexibility to coordinate assessment appointments within required completion timeframes.

Compensation

Compensation is provided as a flat rate per completed in-home assessment, rather than an hourly shift rate.

$100 per visit

  • Supervisory visit ( Done every 60 days)
