LifeChoice Hospice & Palliative Care is seeking an experienced Hospice RN for our overnight on-call team.

During your scheduled week, you will support hospice patients and families by phone and provide in-person care when clinically necessary. A company vehicle will be provided for your entire working week. After seven nights of coverage, you will have seven consecutive days completely off.

What We Do

LifeChoice provides personalized hospice and palliative care to patients in their homes, assisted-living communities, skilled nursing facilities, and other care settings throughout the greater Chicago area.

Our interdisciplinary team manages symptoms, coordinates medications and equipment, supports caregivers, and helps patients remain safe and comfortable. We care for the whole person and support the family throughout every stage of the hospice journey.

What You’ll Do

As the overnight on-call RN, you will be the experienced clinical resource our patients and families can depend on after regular office hours.

You will respond to calls, assess symptoms and changes in condition, provide telephone guidance, and complete in-person visits when needed. You will manage pain and other end-of-life symptoms, provide comfort-medication education, obtain physician orders, and coordinate with pharmacies, facilities, and members of the hospice team.

You will also complete death pronouncements, support families through the next steps, address urgent medication or equipment needs, assist with urgent admissions when needed, and provide clear documentation and reports to the daytime team.

Most importantly, you will bring calm, clinical confidence, and compassionate support into difficult moments.

Schedule

The schedule is 5:30 PM–8:30 AM for seven consecutive nights , followed by seven consecutive days completely off. Weekend and holiday coverage is included when it falls during your scheduled working week. Telephone triage and in-person visits are based on patient needs.

Company vehicle is provided throughout your seven-night working week .

What We’re Looking For

• Active and unrestricted Illinois RN license

• Previous hospice nursing experience is required

• Strong assessment, symptom-management, and end-of-life medication knowledge

• Experience handling after-hours needs and death pronouncements

• Ability to work independently and make sound clinical decisions

• Compassionate communication and timely electronic documentation

• Valid driver’s license and acceptable driving record

• Ability to work the complete 7-on/7-off schedule

Applicants without previous hospice nursing experience will not be considered.
