Role: Registered Nurse in a faith-based community health setting Responsibilities Provide compassionate, holistic care including assessments, care planning, medication administration, vaccines, and patient education Collaborate with medical, dental, behavioral health, and wellness teams to support integrated, whole-person care and promote health equity Coordinate follow-up care and manage chronic conditions Maintain accurate electronic health record (EHR) documentation Uphold safety and quality standards in patient care Model a mission-driven, collaborative culture Qualifications & Skills Current RN licensure Strong clinical assessment and care coordination skills Ability to work effectively within a multidisciplinary team Excellent communication and patient education abilities Commitment to community health and health equity
